how being homeless affects children

Spiritually speaking,Does karma merely mean action or action as well as intentions hidden behind it? Does..?
intentions really affect the results of an action? I mean if two persons are doing same work with different intentions? Will they meet different results?
Suppose two women are running an orphanage, a good cause but the first woman is doing it for fame and putting impression on her friends while the second one is doing just because of her true compassion for homeless children. Will they be admired by God equally or he will also probe into their intentions too before choosing the result of their karma?
Apart form this quesiton, how will you take these three activities karmically
A butcher who kills due to his occupation.
A soldier who kills due to his duty.
A criminal who kills due to his greed, wrong occupation and lack of right guidance.

To continue with your example of these two women.
For the moment they both perform the same actions so they will reap the same results. The difference in their motivation though, may result in a different consciousness in the future.
Righteous motivated people direct their consciousness towards goodness, therefore in the future, even while enjoying the good results, they may continue to perform pious activities and elevate themselves to still higher (heavenly) levels of existence.
Whereas ill-motivated people direct their consciousness towards passion (rajas) and ignorance (tamas), leading to degradation. Therefore, while enjoying the reactions of their good actions, they tend to fall down in perversities and either stay struggling on the earthly platform or go down to the hellish planets.
It is also said that in Kali yuga only actions are taken into account, thoughts do not count for receiving karmic reactions. In the other three ages, one receives karmic reactions for both thoughts and actions.
